Intensity
How much support, how often. The number most commonly inflated in this sector, which is why our recommendations above a set intensity need a second signature from someone with no commercial role.
Why intensity matters.
Intensity is where the commercial interest and the clinical judgement sit closest together, and it is almost never discussed in those terms. A recommendation of twenty hours a week rather than eight is a very large financial decision presented as a clinical one.
That does not make high intensity wrong. Some children genuinely need it. It means the number deserves the same scrutiny you would give any other large purchase: what is it based on, who benefits from it being higher, and what would make it come down.
Questions that get a real answer
- What is this number based on?
- Does anyone's pay change if it goes up?
- What would make you reduce it, and when will we check?
Ask these of anyone, including us. What you are listening for is not the right answer — it is whether the answer is specific enough to be wrong.
